How to Submit a Complaint


The submission form is short and straightforward. This guide walks you through each field so you know what's expected and why we ask for it.

The form, step by step

  1. Go to dancesport.org.au/complaint. Read the brief notes at the top of the form.
  2. Fill in the seven sections (described below). Fields marked with a red asterisk * are required.
  3. Optionally, attach up to 5 PDF files as supporting documents (10 MB each maximum).
  4. Type the security code shown in the CAPTCHA image into the box below it.
  5. Click Submit Complaint.
  6. You'll see a confirmation page with your reference number. A confirmation email will arrive in a few minutes.

Your Full Name (required)

Please enter your full legal name. We use this in our reply and to look up your member record if you're a Member. Pseudonyms and partial names make it harder for us to take your complaint forward.

Email (required)

This is how we'll reply to you. Please double-check that you've typed it correctly — if there's a typo, we can't reach you, and you may think we've ignored your complaint when in fact we have replied.

Tip: Once you submit, you'll get an automatic confirmation email within a few minutes. If you don't see it, check your spam or junk folder before assuming something went wrong.

Phone (optional)

Some matters are best discussed by phone. Include your number if you're comfortable being called, and indicate in the complaint details if there are particular times that suit you.

DSA Member Number (CID) (optional)

If you're a DSA Member, please include your CID. It helps us connect your complaint to your member record and verify that you are who you say you are. If you're not a Member — or you don't know your CID — just leave this blank.

Subject (required)

A short summary of what the complaint is about. Five to ten words is ideal.

Complaint Details (required)

This is where you tell us what happened. The more clearly and factually you can explain the situation, the better we can help. There's no word limit — take the space you need.

A helpful structure to follow:

  • What happened — describe the event or situation factually
  • When and where — dates, times, locations, event names
  • Who was involved — names where you know them
  • Why this is a concern — explain why it troubled you
  • What you would like to see happen — the outcome you're seeking
Stick to what you know. Tell us what you saw, heard, or experienced. If you're describing something that someone else told you, say so. Reporting what you actually know strengthens your complaint — speculation weakens it.

Supporting Documents (optional)

If you have evidence to share — copies of emails, letters, photographs, or other relevant documents — you can attach up to 5 PDF files (10 MB each maximum). See Supporting Documents for guidance on converting things into PDF.

The security code (CAPTCHA)

Before submitting, you'll be asked to type a short code from an image. This protects the form from automated abuse.

If the code is difficult to read, click "Can't read the image? CLICK HERE to refresh" below it to get a new one.

Submitting the form

Once you've filled in everything, click the Submit Complaint button. You'll see a spinning loader for a few seconds while we process the submission and send the confirmation email.

Be patient on the loader screen. If you have attached several large PDFs, the upload can take 30 seconds or more. Please don't close the tab or click Back — you'll see a confirmation page when it's done.
The form does not save drafts. If you close the tab before submitting, you'll need to start over. If you're worried about getting interrupted, type the details into a Word document first and paste them into the form when you're ready.
